Gas Exchange in Humans —

📚 Gas Exchange in Humans |

1. Introduction to Gas Exchange |

All living organisms require a constant supply of oxygen for aerobic respiration and must remove the waste product carbon dioxide. In humans, gas exchange refers to the process by which oxygen is taken up from the environment and carbon dioxide is released. This process occurs across a specialised respiratory surface — the alveolar epithelium in the lungs.

For gas exchange to be efficient, respiratory surfaces must satisfy several key requirements: a large surface area to volume ratio, a thin diffusion pathway, a steep concentration gradient maintained by ventilation and blood flow, and a moist surface for gas dissolution. The human respiratory system has evolved to meet all these demands through an intricate anatomical and physiological design.

2. Structure of the Human Respiratory System |

The human respiratory system is a branching network of airways that progressively narrows from the trachea to the microscopic alveoli. The major structural components include:

2.1 The Conducting Zone | 传导区

Nasal Cavity: Air enters through the nostrils and passes through the nasal cavity, where it is warmed, moistened, and filtered by mucus and cilia. The rich blood supply in the nasal mucosa helps adjust the temperature of incoming air to body temperature.

Pharynx and Larynx: The pharynx (throat) serves as a common pathway for both air and food. The larynx (voice box) contains the vocal cords and acts as a gateway to the trachea. The epiglottis, a flap of cartilage, closes over the larynx during swallowing to prevent food from entering the airway.

Trachea: The trachea (windpipe) is a flexible tube approximately 10-12 cm in length, reinforced by C-shaped rings of hyaline cartilage. These rings prevent the trachea from collapsing during inhalation while still allowing the oesophagus behind it to expand during swallowing. The inner lining is composed of ciliated pseudostratified columnar epithelium with numerous goblet cells that secrete mucus to trap dust particles and pathogens.

Bronchi and Bronchioles: The trachea divides into the left and right primary bronchi, which enter the lungs at the hilum. Inside the lungs, the bronchi branch repeatedly into secondary and tertiary bronchi, then into bronchioles. As the airways narrow, the cartilage is gradually replaced by smooth muscle, which allows for bronchoconstriction and bronchodilation — important regulatory mechanisms for airflow.

2.2 The Respiratory Zone |

Respiratory Bronchioles: These are the first structures where gas exchange begins to occur. They have thin walls with occasional alveoli budding from their surfaces.

Alveolar Ducts and Alveoli: The respiratory bronchioles lead into alveolar ducts, which terminate in clusters of alveoli called alveolar sacs. Each lung contains approximately 300-500 million alveoli, providing a combined surface area of roughly 70-100 m² — approximately the size of a tennis court. This enormous surface area is critical for efficient gas exchange.

Distribute air to each lung; contain cartilage and smooth muscle

3. The Alveolar Wall: Adaptations for Gas Exchange |

The alveolar wall is exquisitely adapted for its gas exchange function. Each alveolus is lined by a single layer of squamous epithelial cells, making the diffusion distance between alveolar air and capillary blood as short as 0.2-0.5 μm. This is one of the thinnest barriers in the human body.

3.1 Key Cell Types in the Alveolus |

Type I Pneumocytes (Squamous Epithelial Cells) I型肺泡细胞 Extremely thin, flattened cells covering ~95% of the alveolar surface 极薄的扁平细胞,覆盖约95%的肺泡表面 Provide minimal diffusion distance for gas exchange 为气体交换提供最短扩散距离
Type II Pneumocytes (Septal Cells) II型肺泡细胞 Cuboidal cells scattered among Type I cells 散布在I型细胞之间的立方形细胞 Secrete pulmonary surfactant to reduce surface tension and prevent alveolar collapse
Phagocytic cells on the alveolar surface or in the interstitium 位于肺泡表面或间质中的吞噬细胞 Engulf foreign particles and pathogens; provide immune defence 吞噬外来颗粒和病原体;提供免疫防御

3.2 Pulmonary Surfactant |

Type II pneumocytes secrete a mixture of phospholipids and proteins known as pulmonary surfactant. This substance reduces the surface tension of the fluid lining the alveolar surface, which is essential for two reasons. First, it prevents the alveoli from collapsing during exhalation — a condition known as atelectasis. Second, it reduces the work required to inflate the lungs during inspiration. The importance of surfactant is dramatically illustrated in premature infants born before 28 weeks of gestation, who may develop Neonatal Respiratory Distress Syndrome (NRDS) due to insufficient surfactant production.

4. The Mechanism of Ventilation |

Ventilation is the process by which air moves into and out of the lungs. It maintains the concentration gradients of oxygen and carbon dioxide between the alveolar air and the blood. Ventilation consists of two phases: inspiration (inhalation) and expiration (exhalation).

4.1 Inspiration (Active Process) |

Inspiration is an active process that requires energy. The diaphragm, a dome-shaped sheet of skeletal muscle separating the thoracic cavity from the abdominal cavity, contracts and flattens. Simultaneously, the external intercostal muscles between the ribs contract, pulling the rib cage upwards and outwards. These coordinated muscle contractions increase the volume of the thoracic cavity, which decreases the intrapulmonary pressure below atmospheric pressure. Air rushes into the lungs down this pressure gradient until the pressures equalise.

4.2 Expiration (Passive at Rest) |

During quiet breathing, expiration is a passive process. The diaphragm and external intercostal muscles relax. The elastic recoil of the lung tissue and the surface tension of the alveolar fluid cause the lungs to shrink back to their resting volume. As the thoracic cavity volume decreases, the intrapulmonary pressure rises above atmospheric pressure, forcing air out of the lungs.

During forced expiration — such as during exercise, coughing, or sneezing — the internal intercostal muscles and abdominal muscles contract to push air out more forcefully, making expiration an active process.

5. Gas Exchange at the Alveolar-Capillary Interface |

Gas exchange occurs by simple diffusion across the respiratory membrane. This membrane is composed of three layers: the alveolar epithelial wall, the fused basement membrane of the alveolar and capillary endothelia, and the capillary endothelial wall. The total thickness of this barrier is less than 1 μm, making it ideally suited for rapid diffusion.

5.1 Fick’s Law of Diffusion |

The rate of gas diffusion across the respiratory membrane is described by Fick’s Law:

Rate of Diffusion ∝ (Surface Area × Concentration Gradient) / Diffusion Distance

This equation highlights why the alveolar structure is so effective: the enormous surface area of the alveoli maximises the numerator, while the extreme thinness of the respiratory membrane minimises the denominator. Furthermore, the continuous ventilation of alveoli and perfusion of pulmonary capillaries maintain steep concentration gradients for both O₂ and CO₂.

5.2 Partial Pressure Gradients |

The direction and rate of gas movement are determined by partial pressure differences. The partial pressure of a gas in a mixture is the pressure that gas would exert if it alone occupied the entire volume. Gas molecules diffuse from regions of higher partial pressure to regions of lower partial pressure.

Atmospheric Air
Alveolar Air
Oxygenated Blood (Pulmonary Veins)
Tissue Cells
Deoxygenated Blood (Pulmonary Arteries)

In the lungs, oxygen diffuses from alveolar air (pO₂ = 100 mmHg) into deoxygenated blood (pO₂ = 40 mmHg) across a gradient of approximately 60 mmHg. Simultaneously, carbon dioxide diffuses from deoxygenated blood (pCO₂ = 46 mmHg) into alveolar air (pCO₂ = 40 mmHg). At the tissues, the gradients are reversed: oxygen diffuses from oxygenated blood into respiring cells, and carbon dioxide diffuses from the cells into the blood.

6. Oxygen Transport in the Blood |

Only about 1.5% of oxygen is transported dissolved in the blood plasma. The remaining 98.5% is transported reversibly bound to haemoglobin (Hb), the protein pigment found within red blood cells (erythrocytes). Each haemoglobin molecule consists of four polypeptide subunits — two alpha and two beta chains — each containing a haem group with an iron atom (Fe²⁺) at its centre. Each iron atom can bind one O₂ molecule, meaning each haemoglobin molecule can carry up to four oxygen molecules.

6.1 The Oxygen-Haemoglobin Dissociation Curve |

The binding of oxygen to haemoglobin is cooperative. The binding of the first O₂ molecule causes a conformational change in the haemoglobin molecule, making it easier for subsequent O₂ molecules to bind. This produces the characteristic sigmoidal (S-shaped) oxygen-haemoglobin dissociation curve.

At the high pO₂ found in the lungs (~100 mmHg), haemoglobin is nearly 100% saturated with oxygen. At the lower pO₂ in respiring tissues (~40 mmHg), haemoglobin releases approximately 25-30% of its bound oxygen — a quantity sufficient to meet metabolic demands while retaining a reserve for periods of increased activity.

6.2 The Bohr Effect |

The Bohr effect describes how increased pCO₂ and decreased pH shift the oxygen-haemoglobin dissociation curve to the right. In actively respiring tissues, CO₂ production increases, forming carbonic acid and lowering pH. This rightward shift reduces haemoglobin’s affinity for oxygen, promoting oxygen unloading exactly where it is most needed. The Bohr effect thus represents an elegant physiological adaptation that links oxygen delivery to metabolic demand.

7. Carbon Dioxide Transport |

Carbon dioxide is transported in the blood by three main mechanisms:

Dissolved in Plasma 溶解于血浆 ~7% CO₂ dissolves directly in the blood plasma 二氧化碳直接溶解于血浆中
Carbaminohaemoglobin 氨基甲酸血红蛋白 ~23% CO₂ binds reversibly to amino groups on the globin chains of haemoglobin 二氧化碳与血红蛋白珠蛋白链上的氨基可逆结合
Bicarbonate Ions (HCO₃⁻) 碳酸氢根离子 ~70% CO₂ reacts with water inside red blood cells, catalysed by carbonic anhydrase, forming H₂CO₃ which dissociates into H⁺ and HCO₃⁻ 二氧化碳在红细胞内与水反应,由碳酸酐酶催化,形成H₂CO₃,再解离为H⁺和HCO₃⁻

The conversion of CO₂ to bicarbonate inside red blood cells is the predominant transport mechanism and relies on the enzyme carbonic anhydrase. The bicarbonate ions are then exchanged for chloride ions across the red blood cell membrane — a process known as the chloride shift — which helps maintain electrical neutrality. At the lungs, the reaction reverses: bicarbonate re-enters the red blood cells, combines with hydrogen ions to form carbonic acid, which is then converted back to CO₂ and H₂O for exhalation.

8. Control of Breathing |

Breathing is controlled by the respiratory centre located in the medulla oblongata and pons of the brainstem. This centre generates the rhythmic pattern of inspiration and expiration, with a basic rhythm that can be modified by both chemical and neural inputs.

8.1 Chemical Control |

The primary stimulus for breathing is the partial pressure of carbon dioxide in the blood rather than oxygen. Central chemoreceptors in the medulla detect changes in the pH of cerebrospinal fluid, which reflects arterial pCO₂. When pCO₂ rises, more carbonic acid forms, lowering pH, and the respiratory centre increases both the rate and depth of breathing to expel excess CO₂.

Peripheral chemoreceptors located in the carotid bodies (at the bifurcation of the common carotid arteries) and the aortic bodies (in the aortic arch) are sensitive to decreases in arterial pO₂, increases in pCO₂, and decreases in pH. These receptors become particularly important when arterial pO₂ falls below approximately 60 mmHg — a level at which haemoglobin saturation drops sharply.

8.2 Neural Control |

The respiratory centre receives input from higher brain centres, allowing voluntary control of breathing — for example, when speaking, singing, or holding one’s breath. Stretch receptors in the bronchi and bronchioles detect lung inflation and trigger the Hering-Breuer reflex, which inhibits further inspiration to prevent over-inflation of the lungs.

9. Lung Volumes and Capacities |

Lung function can be assessed by measuring various volumes and capacities using a spirometer:

Volume of air inhaled/exhaled in a single normal breath
Maximum additional volume that can be inhaled after a normal inhalation
Maximum additional volume that can be exhaled after a normal exhalation
Volume of air remaining in the lungs after maximal exhalation; prevents lung collapse
Vital Capacity (VC = TV + IRV + ERV) 肺活量 Maximum volume exhaled after maximum inhalation 最大吸气后能呼出的最大气量 ~4600 mL
Total Lung Capacity (TLC = VC + RV) 肺总量 Total volume of air in the lungs after maximum inhalation

10. Common Respiratory Disorders |

Understanding the normal physiology of gas exchange provides a foundation for comprehending pathological conditions:

Asthma: A chronic inflammatory condition characterised by bronchoconstriction, mucus hypersecretion, and airway remodelling. During an asthma attack, the smooth muscle in the bronchioles contracts excessively, narrowing the airways and increasing resistance to airflow. This makes expiration particularly difficult, leading to wheezing and shortness of breath.

Chronic Obstructive Pulmonary Disease (COPD): A progressive disease primarily caused by smoking, characterised by the destruction of alveolar walls (emphysema) and chronic inflammation of the airways (chronic bronchitis). The loss of elastic tissue and alveolar surface area severely impairs gas exchange efficiency.

Pulmonary Fibrosis: The formation of scar tissue in the lungs, which thickens the respiratory membrane and increases the diffusion distance for gases. This reduces the rate of gas exchange and leads to breathlessness even at rest.
