Act utilitarianism directly evaluates each individual action, while rule utilitarianism would consider whether a rule permitting sacrificing one to save many would lead to greater utility in the long run.

In contrast, Kantian deontology would forbid pulling the lever. Kant’s categorical imperative demands that we treat humanity always as an end in itself, never merely as a means. Moreover, the first formulation – act only according to that maxim whereby you can at the same time will that it should become a universal law – might be tested: could you rationally will a world where innocent people are sacrificed for the greater good? Kant would argue such a world is contradictory because the very notion of treating persons as ends would be undermined.
